Our funding arrangements range from 100% employee-paid to 100% employer-paid.

We offer a wide variety of dental benefit plan designs and funding arrangements for employers of all sizes. We are continually integrating clinical research, consumer-value approaches and industry trends into our standard plans to offer solutions to help you meet your goals.

Dental benefit plans are available to employers based upon group size, underwriting and state requirements.

 

The MetLife Preferred Dentist Program (PDP) – The MetLife PDP, our Preferred Provider Organization (PPO), is the cornerstone upon which we have built a wide range of dental benefit options.

  • More than 110,000 dentist locations, including more than 25,000 specialist locations, participate in the PDP nationwide

The PDP provides freedom of choice. Employees may go out-of-network at any time and still receive benefits. Employees may pay more out-of-pocket for the services of non-network dentists, but the choice remains theirs.

 

PDP Copay Plan – With flexible plan design options and monthly premiums as low as $8–$12, the MetLife PDP Copay Plan can help lower your dental costs over time, while keeping your employees satisfied. Participants’ out-of-pocket costs are presented as a set dollar value rather than a coinsurance percentage — making it easy for employees to understand while allowing you to increase cost sharing without dissatisfaction (e.g., $5 prophylaxis “cleaning” instead of a 95% coinsurance benefit).

 

Graduating Dental Benefits Graduating Dental Benefits from MetLife encourages participants to maintain their dental coverage and provides enhanced coverage to help ALL participants achieve and/or maintain good oral health. Participants, including dependents, are rewarded for maintaining their dental coverage with MetLife with an increasing annual maximum benefit4 each year upon the participant’s anniversary with MetLife dental coverage for up to three years5,6 — only requirement is participants maintain enrollment (no gaps) in the MetLife dental plan.

  • Graduating Dental Benefits is easy for employees to understand and enhances the value of an employer’s dental plan offering.

Full Service Dental for Retirees Our Full Service Dental for Retirees was designed to provide employers with a turnkey way of satisfying retirees’ need for reasonably priced dental benefits, while giving employees the assurance of knowing that, when they retire, they continue to have access to dental benefits.

  • With Full Service Dental for Retirees, employers can enrich their retiree benefits with minimal administration and no benefit expense to their company. MetLife handles marketing, enrollment, customer service, recordkeeping and billing. Retirees pay the entire premium.

Dental Health Maintenance Organization (DHMO) – With MetLife’s recent acquisition of SafeGuard Health Enterprises, Inc. (SafeGuard), we now offer access to DHMO plan options in California, Florida and Texas.

 

These new DHMO options, offered through SafeGuard, a MetLife company, deliver an extensive range of cost-effective plan choices with broader coverage than many traditional DHMO plans and a robust network from which employees can choose.

Indemnity Plans – MetLife also provides a traditional fee-for-service dental plan giving participants the freedom to visit any licensed dentist, no network, in the country for covered services.

Like most group dental policies, MetLife group policies contain certain exceptions, waiting periods, reductions, limitations and terms for keeping them in force. Please contact us for costs and complete details.

 

Group dental insurance policies featuring the Preferred Dentist Program (PPO) are underwritten by Metropolitan Life Insurance Company, New York, NY 10166. DHMO plans are available through subsidiaries of SafeGuard, a MetLife company, as follows: in California, SafeGuard Health Plans, Inc., a California corporation; in Florida, the plans are known as Dental Managed Care Plans provided by SafeGuard Health Plans, Inc., a Florida corporation; and in Texas, SafeGuard Health Plans, Inc., a Texas corporation.

1MetLife PDP Copay plans are only available to groups with 500 or more eligible employees. Copay plans are not available for insured cases in Texas, including plans sitused in Texas or covering Texas residents. Copay plans are also not available for insured cases in Connecticut.

2Available for groups with 10 or more eligible employees, excluding Copay plans and Full Service Dental for Retirees.

3Self-funded programs are available to organizations with 500 or more eligible employees. DHMO plans are not offered on a self-funded basis.

4At this time, each increment to the annual maximum can be $250 for in-network and out-of-network or $500 for in-network only.

5Exact time frames are determined by the employer. Assuming no gap in MetLife dental coverage with the same employer, the annual maximum will graduate (increase) for employees and their covered dependents until reaching the maximum annual benefit. The increase occurs on the anniversary (12 months of coverage) of when the participant’s coverage became effective under the plan. MetLife dental coverage refers to dental plans underwritten or administered by MetLife.

6The highest annual maximum level is capped at three years or $3,000.
