But with over 135,000 in-network dentist locations—including over 32,000 specialist locations—there’s a good chance your dentist is part of the MetLife PDP. Plus, you’ll be assured they’ve passed a rigorous selection process.

There’s no paperwork if your dentist submits your claims for you. We can even give you pre-treatment estimates while you’re at the dentist. Plus, any time you want to check coverage, claim status or history, or get an estimate, you or your dentist can get a quick answer by phone, fax or online, so you can get the information you want, when you want it.

A participating PDP dentist is a general dentist or specialist who participates in MetLife’s Preferred Dentist Program (“PDP”) who meets MetLife’s well-established credentialing standards and agrees to accept negotiated fees as payments-in-full for services rendered. When you visit a participating dentist, you are responsible only for the difference between what the plan benefit pays and the negotiated fee which is typically 15-45% below the average charge of dentists for the same services in a given geographical area.

Yes, you can go to the dentist you’re most comfortable with, even if he or she isn’t in our network, and still receive benefits. Please keep in mind that visiting a dentist who is not in our network may result in higher out-of-pocket costs since the PDP negotiated fees do not apply to out-of-network services. This means you will be responsible for any difference between the dentist's usual fee and your plan's payment. Plans vary, so examine the specifics of your company’s benefit package for deductibles, annual maximums and frequency limits to help determine actual out-of-network benefits.

Dentists may submit your claims for you, which means you have little or no paperwork. You may also download a Dental Claim form. Just fill out the employee portion and have your dentist complete the rest. Either you or your dentist can submit the form, and benefits are paid to you, unless you’ve arranged for them to go directly to your dentist. Your completed claim form should be mailed to:

You can track your claims online and even receive e-mail alerts when a claim has been processed. Once your claim is processed, you’ll receive an Explanation of Benefits statement.

We recommend that you receive a pre-treatment estimate through your dentist before more costly service is provided. It is the best way to determine your estimated out-of-pocket expense before services are provided.
