Download your Schedule of Benefits (SOB)
The information about your benefits, covered services, and how much they cost is outlined in your Schedule of Benefits. Each employer picks different benefits, benefit levels, and services for their employees (you!), so every Schedule of Benefits is different. We can't post all Schedule of Benefits here on this page, so we post them securely in our website, eBenefits. To get yours, login to eBenefits and:
- Click the “Documents and Forms” tab.
- In the Category box, select “Policy Documents” from the dropdown.
- In the Subcategory box, select “Schedule of Benefits” from the dropdown.
- Click the Search button.
Login to eBenefits now to download your Schedule of Benefits.
What
The key things you should look for in your Schedule of Benefits are your deductible, coinsurance, and out-of-pocket maximum. Keep in mind, these may vary depending on your location; for example, they may be different internationally vs. in the US.
Deductible
- A flat amount that an insure must pay before the insurance company will make any benefit payments under a health insurance policy.
Coinsurance
- The amount of eligible expenses the insured is responsible for paying after any applicable deductibles are met.
Out-of-pocket maximum
- The most you must pay out-of-pocket until the plan pays for everything at 100%.
