  • This plan provides protection for covered events1 experienced while off the job only. This means that any accidents that occurred while at work, would not be covered.

Here's how it works:

Kathy’s daughter, Molly, plays soccer. During a recent game, Molly collided with an opposing player, was knocked unconscious and taken to the local emergency room (ER) by ambulance for treatment. The ER doctor diagnosed a concussion and a broken tooth. He also ordered a CT scan. After thorough evaluation, Molly was released to her primary care physician for follow-up treatment, and her dentist repaired her broken tooth with a crown. * Notes Regarding Certain Benefits

  • Fracture and Dislocation benefits – Chip fractures may be paid at a reduced percentage of the Fracture Benefit and partial dislocations may be paid at a reduced percentage of the Dislocation Benefit.
  • Hospital Benefits – "Hospital" does not include certain facilities such as nursing homes, convalescent care or extended care facilities. Please consult your certificate for details.
  • Hospital Admission and Confinement – The Admission Benefit is not payable for Emergency Room treatment or outpatient treatment. The payment of the admission benefit requires a Confinement. Hospital Confinement requires the assignment to a bed as a resident inpatient in a Hospital (including an Intensive Care Unit of a Hospital) on the advice of a Physician or confinement in an observation area within a Hospital for a period of no less than 20 continuous hours on the advice of a Physician. Please consult your certificate for details.
  • Lodging Benefit – The lodging benefit is not available in all states. It provides a benefit for a companion accompanying a covered insured while hospitalized, provided that lodging is at least a certain number of miles from the insured’s primary residence as defined in the certificate.

Regular full-time and part-time employees working in the United States are eligible to enroll in Personal Accident Insurance (no monthly minimum hourly requirement). You may enroll yourself and your eligible family members.7 You need to enroll during your Enrollment Period and be actively at work for your coverage to be effective.

While over 150 accidents are covered under this plan, accidents or injuries occurring at or due to your occupation are not covered. Workers compensation may offer protection in these instances. Refer to your certificate of coverage or outline of coverage for a comprehensive list of covered accidents.

To file an Accident claim, you will need to provide the following:

  • Certificate holder information - name, certificate number, address, date of birth, social security number and contact Information
  • Patient information for claimant – such as dependent status, name, address, date of birth, gender, social security number, and contact information
  • Accident details – such as when and where accident occurred, description of accident/injury, if motor vehicle or police report was involved, and provider name and contact information
  • Payment instructions

You can find your certificate number by logging in to MyBenefits or by calling Amazon’s designated team at 800-638-2880. To locate your certificate in MyBenefits, once logged-in, choose benefit under the “My Accounts Page.” Locate any correspondence and/or your certificate under the “Correspondence” tab. When calling MetLife, please be prepared to provide personal information such as name, social security, personal identification number or address.

Medical information needed is dependent upon accident, however may consist of: itemized invoices/bill, admission and discharge papers from hospitals, treatment notes, x-rays, treatment plan details, referral details, death certificate, autopsy, blood tests for alcohol and drug screening for motor vehicle accidents, hospital and rehab invoices and notes, orthopedic notes and information on assistive devices/durable medical equipment, and/or any other medical information pertinent to the claim.

Please provide any available supporting medical documentation with your claim. If any additional medical documentation is needed, MetLife will assist in obtaining this information from your provider.

Yes, you can take your coverage with you.8 You will need to continue to pay your premiums to keep your coverage in force. Your coverage will only end if you stop paying your premium or if your employer offers you similar coverage with a different insurance carrier.
