Here’s what you need to know about taking leave

Whether you require Short-Term or Long-Term Disability benefits or seek support through the Family and Medical Leave Act (FMLA), Caregiver Leave or a State Mandated Plan please notify your manager the first date you intend to take off. If your leave request is the result of a sudden occurrence, file your claim as soon as possible.

Before taking leave, you’ll need to provide MetLife with supporting medical documentation for you or your family member, except where prohibited by law. MetLife’s MyBenefits portal makes it easy to submit a claim, upload necessary documentation, and track the claim’s status. Ready to return to work? Let your manager know your anticipated end date and inform MetLife when you’re back so they can close your claim.
