Life and AD&D Death Claims:
To File a Claim
The beneficiary should contact the GIC at 617-727-2310
The Group Insurance Commission (GIC) wants to process the beneficiary’s claim as soon as possible. A family member or a close friend should contact the GIC and provide the following information:
- A copy of the certified death certificate to initiate the claim.
- A copy of the insured’s life insurance certificate or GIC annual benefit statement (if available).
- All beneficiary names, dates of birth, Social Security numbers and addresses.
The GIC will process this information and forward it to MetLife. MetLife will then send a claim package which will include a Claim Form to the beneficiary. The beneficiary must complete the Claim Form and return it to MetLife with a copy of the death certificate. The beneficiary will be able to complete the claim submission online or via hard copy. Upon receipt, MetLife will send a claim payment to the beneficiary. This procedure applies for all deaths, regardless of cause. For Accidental Death and Dismemberment, MetLife may request additional information such as police reports and/or any related newspaper or verifiable online articles. Any additional information requested for an AD&D claim must be received by MetLife within 365 days of the date of death.
- Accelerated Benefit Option (ABO): The member should contact the GIC to report the ABO claim. The GIC will gather the necessary information and submit the claim to MetLife. Upon receipt of the claim information from the GIC, MetLife will send an ABO claim package to the member and the member will submit the completed claim package directly to MetLife for processing.
- AD&D Non-Death Claim: The member should contact the GIC to report the AD&D claim. The GIC will gather the necessary information and submit the claim to MetLife. Upon receipt of the claim information from the GIC, MetLife will send an AD&D claim package to the member and the member will submit the completed claim package directly to MetLife for processing.
- Life Insurance Waiver of Premium (member enrolled in LTD): If the member is on a GIC MetLife approved LTD claim and they meet the Waiver of premium eligibility criteria (disability incurred prior to age 60 and the waiver claim is being filed within 2 years of the date of disability) the employee will not have to call the GIC to report the Waiver claim and a claim form will not be required. MetLife will review the medical information available with the LTD claim as well as Life Insurance information provided separately by the GIC to adjudicate the Waiver claim. MetLife will send a letter to the employee and GIC notifying them of the Waiver of Premium claim decision.
- Life Insurance Waiver of Premium (member not enrolled in LTD): The member should contact the GIC to report the Waiver claim. The GIC will gather the necessary information and submit the claim to MetLife. Upon receipt of the claim information from the GIC, MetLife will reach out to the applicable Agency Coordinator to gather the following member information:
- Date of Hire
- Date of Birth
- Occupation/Job Title
- Hourly/Salaried
- Date Last Worked
Upon receipt of the Agency Coordinator information, MetLife will confirm the member’s eligibility for Waiver of Premium and if eligible will send a Waiver of Premium claim form to the member. The member should complete the claim form and return it to MetLife for review. MetLife will send a notification informing the member and the GIC whether the claim is approved.
Absolute Assignment: The member should contact the GIC to initiate the absolute assignment process. MetLife will send the member an Absolute Assignment form which the member should complete and return to MetLife. MetLife and the GIC will review the form and if satisfactory the Absolute Assignment will be approved.
