A claim needs to be filed with MetLife upon the death of the insured. Please call MetLife customer service at 866-548-7139. The claim form will be sent to the beneficiary on file. Proceeds are paid to your beneficiary through a tax-free death benefit. If you do not designate a beneficiary, claim payment will be distributed following the facility of payment provisions outlined in the certificate of insurance. If the total amount of your combined life benefits is $100,000 or less, and if MetLife can verify the death through other means, the benefit may be paid without submission of a death certificate. Please note this does not apply to dependent claims, AD&D claims, foreign claims, or claims with certain causes of death.
