STEP 1: Please have the following information readily available:
- Your Social Security Number
- Your beneficiary’s Social Security Number, birth date and address
- Your Plan Number for the product you select:
- Your Mutual Fund Select Portfolios (MFSP) Plan Number: 1013891-02
- Your MetLife Financial Freedom Select® (MFFS) Plan Number: 0836786

STEP 3: IMPORTANT FINAL STEP
In order to process your enrollment, please be sure to complete the Milwaukee Public Schools 403(b) Plan Salary Reduction Agreement. Once completed, your Salary Reduction Agreement form needs to be submitted for processing either via email or fax to:
