AFGE offers you the opportunity to enroll in a DHMO dental plan. Below are highlights of your plan to help you determine if a DHMO plan meets your needs. See your Schedule of Benefits documents at the bottom of the chart for complete benefit details.

To enroll in the DHMO plan, complete an enrollment form with a direct deposit document and submit those to your Benefit Architects Administrator or contact Benefit Architects for more information at 1-800-733-7236, extension 105. Click on the appropriate link for a complete enrollment packet.
