Updating beneficiary information

For beneficiaries

It's a good idea to review your life insurance beneficiary designations to ensure your information is current.

Do any of these statements pertain to you?

Were you recently married or divorced?
Have you just purchased a new home?
Do you have children or grandchildren who will soon start college?

Even if the above statements don’t apply to you, you should take a few minutes to review your life insurance beneficiary designations. This ensures MetLife will distribute the policy proceeds the way you want after your death.

You can change your life insurance beneficiaries at any time through your MyBenefits with PEBA. The change is effective on the date the request was made. Please note MetLife will allow beneficiary changes by power of attorney only if the documents specifically state an attorney-in-fact has the power to change beneficiary designations.

We are sorry for your loss and understand this could be a very difficult time for you. We are here to help make submitting your life claim a bit easier.

When the insured or dependent dies, their employer should be notified. This should be done as soon as reasonably possible. The benefits administrator will notify PEBA and PEBA will submit the information to initiate a life insurance claim to MetLife. PEBA will send the beneficiary a condolence letter and a beneficiary statement requesting an original certified death certificate. The beneficiary should mail or fax the beneficiary statement and original certified death certificate to MetLife.

Alternatively, the beneficiary has the option to complete the Claimant Statement online by visiting metlife.com/lifeclaims. Beneficiaries must first register and create a personal username and password. Once registered, the beneficiary will be prompted to enter these access codes:

Access Code

Beneficiaries should be sure to safeguard these codes as they would any important online passwords. Please contact the insured’s employer if these fields are blank.

Please note: the designated beneficiary will be required to enter the insured’s Social Security number. The beneficiary will not be able to proceed without entering that number, which may be found on the death certificate.
