Hospital Indemnity Insurance FAQs

UFA members can claim Hospital Indemnity Insurance benefits from MetLife by completing a claim form and submitting all requested information.  The claim form can be found on this website by clicking the link for Claim Form. 

At your option, you can submit your claim online and upload supporting information to our website.  To submit a claim online, visit mybenefits.metlife.com or download the MetLife Mobile App.

Coverage details can be found by clicking on the link on this website that is titled Coverage Details.  This will bring up an Outline of Coverage that will detail the plan’s benefits including limitations and exclusions. Please refer to the Certificate of Insurance for complete plan details.

While MetLife does not require a beneficiary be designated for Accident Insurance, Critical Illness Insurance and Hospital Indemnity Insurance, we highly encourage you to do so. To designate a beneficiary, download the form in this tile called Beneficiary Designation Form. Complete and mail the form to MetLife at the address noted on the form. You need only complete one form for all three products in the bundle, Accident Insurance, Critical Illness Insurance and Hospital Indemnity Insurance. You will need to complete a separate beneficiary form for your Life/AD&D Insurance and Disability Insurance with Winston Benefits during enrollment for these benefits.
