Life Insurance FAQs

To initiate Life and AD&D claims, contact Winston Benefits at 1-800-347-6071.  Winston Benefits will send the beneficiary form directly to the beneficiary(ies) on record.  This form is to be completed and submitted directly to MetLife at the address shown on the form along with the supporting information requested.

Beneficiary(ies) will provide the completed Life Claim packet to MetLife including all required supporting documents.  Supporting documents include the claimant affidavit, death certificate and certification of trust assignment form (if applicable).

For Accidental Death & Dismemberment claims, a separate form will be provided by Winston Benefits for submission to MetLife.
