You can obtain your withdrawal form online at mlr.metlife.com. The form should be completed and sent per the instructions on the form. The DCSD Human Resources department will review your form and approve the transaction.  The form should be processed within 5-7 days.

You can request a loan online at mlr.metlife.com. Loan repayments are made through ACH from participants’ bank accounts.

You can obtain a Hardship distribution form online at mlr.metlife.com. The form should be completed and sent in with all the appropriate Hardship documentation per the instructions on the form.  The DCSD Human Resources department will review your form and approve the transaction.

Before you begin, please have your beneficiary’s name, tax ID or social security number, and address handy. To name or update your beneficiary, log on to mlr.metlife.com and click on “My Accounts” to begin.  Also, don’t forget to name a contingent beneficiary.  Please read this flyer for more information on the importance of keeping your beneficiary designations up-to-date.
