2 Custom LASIK coverage only available using wavefront technology with the microkeratome surgical device. Other LASIK procedures may be performed at an additional cost to the member. Laser vision care discounts are only available from in-network contracted facilities.
3 Not all Pearle Vision locations participate in the MetLife Vision program.
4 Some groups utilize Walmart and Sam’s Club as in-network providers. Once enrolled, check the provider listing in MyBenefits (www.metlife.com/mybenefits) to confirm whether Walmart is considered in-network or out-of-network.
