Beneficiary support
We offer tools and services to support you and your loved ones during an accidental death or a dismemberment.
Accidental Death & Dismemberment (AD&D) claims
If you pass away, the AD&D claim will be paid automatically with your Life insurance claim, so there are no additional steps required for your beneficiary or employer. In the event of a dismemberment, you will need to submit a claim to MetLife for payment.
Here’s what’s needed for a dismemberment claim:
Life Claim Form
Collects information about your policy
Reimbursement form
Captures payment preferences and banking information
Relevant medical records
Documentation relating to the injury, including hospital and operative records, police or incident reports, or health records.
Claim forms
The Life Claim and Reimbursement forms can be completed digitally and submitted via email or fax. Forms are available on our Claim forms page.
What happens next
Once the claim is submitted:
- Processing – We review the claim. If additional information is needed, we’ll contact the submitter. Our goal is to process claims within 10 business days once all documentation is received.
- Confirmation – The submitter will receive an email once the claim is finalized.
- Payment – Funds are issued based on the preferences in the Reimbursement Form. Timing may vary depending on banking systems, especially for international payments.
