Our Claim and Reimbursement forms are available from the dropdown below. You will need to complete one of each as follows:
- Claim forms capture the details of your policy with MetLife. Please select the appropriate form from the dropdown below (Life, Long Term Disability, or Medical).
- Reimbursement form captures your payment preferences and banking information. Please select “Reimbursement Forms” from the dropdown below to download.
Forms may have different information and requirements based on the insurer. Your insurer is either Delaware American Life Insurance Company (DelAm) or Voyageur Global Benefits (VGB), depending on your employer. U.S.-based employers typically use DelAm; Canadian employers usually use VGB. To confirm, check the disclaimer on your ID card or your Schedule of Benefits.
All forms can be completed digitally and submitted via email (or fax). Please submit both the Claim form and Reimbursement form to us to expedite processing.
