Requesting a FastFax

General information about the details of your benefits will always be available to you and your dentist. If you have questions during your next visit, your dentist should be able to provide you with the information easily.

You can:

Call 1-800-942-0854, follow the automatic prompts and ask to have a ‘FastFax’ sent directly to you
If you don’t use your social security number for insurance information,
call 1-866-832-5756 follow the automatic prompts and ask to have a ‘FastFax’ sent directly to you

You can ask your dentist to:

go online to www.metdental.com and select ‘Eligibility and Plan Design’
call 1-877-MET-DDS9 (1-877-638-3379) and request a ‘FastFax’ of your dental plan information
