  3. Which options are necessary, elective, or cosmetic?
  4. What are the fees for the recommended treatment?
  5. Which of the fees are covered by my insurance and which are not?
  6. Are the fees reasonable?
  7. Do I need to ask the dentist to submit the treatment plan for a pretreatment estimate of benefits so I know what to expect?
  8. Is my dentist a member of my plan's network?
