We offer a Preferred Provider Organization (PPO) individual dental insurance policy with competitive prices and great benefits, including:

We also will soon be offering a Dental HMO/Managed Care program in select states with a broad network and over 400 services covered. (DHMO/Managed Care will be available to residents of CA, FL, NY, and TX only)

Dental Health Maintenance Organization (DHMO)/Managed Care –
  will only be available to individuals residing in California, Florida, Texas and New York. This program is designed to help you and your family maintain oral health and reduce your out-of-pocket costs. Also included are other valuable features, such as over 400 covered services, no deductibles and no claim forms, and a broad network of participating general dentists and specialists. How would I pay for coverage?

Premiums can be paid by check, credit card or bank draft. At time of enrollment, you must elect which method of payment you prefer.

Can I enroll my dependents for dental coverage?

Yes. At time of enrollment, you are able to elect coverage for your dependents. If you decide after the policy takes effect that you would like to enroll additional dependents for coverage, you will need to provide MetLife with advanced written notice along with any required premium to add the dependent(s). After we receive your written notice to add dependents, your premium rates will be adjusted as of the date coverage takes effect for the newly added dependent(s). The effective date of coverage for newly added dependent(s) will depend on when we receive notice and required premium.
